 Information for Public Schools Regarding Changes in Health Examination and Screening Requirement Effective 7/1/2018


1. Section 136.3 regarding Health Examinations and Screenings will change. The new regulations for health examinations for attendance will be required for new entrants and in grades Pre-K or K, 1, 3, 5, 7, 9, and 11. 

2. The new regulations in vision and hearing screenings will be required within 6 months of admission to school and in grades Pre-K or K, 1, 3, 5, 7, and 11. Scoliosis screenings will be required for girls in grades 5 and 7 and for boys in grade 9. 

3. The only form that may be used to document health examinations for students attending public school is the approved form titled; NYSED (New York State Education Department) Student Health Examination Form.
The new NYS School Health Examination Form is not required for physicals done prior to 7/1/18. 
To assist with the transition, during the 2018-2019 school year schools may accept any health examination form provided.  
In 2019-2020 ONLY the approved form will be accepted.
